To start the observation step, a provider submits her Request for Observation Visit. This packet of information includes the following required documents:
  • Request for Observation Visit Form
  • Balance of NAFCC Accreditation Fees
  • Enrollment Information Form
  • Current License
  • NAFCC Health Assessment Form
  • NAFCC TB Screening Form
  • Current First Aid
  • Current Pediatric CPR
  • State Background Check
  • FBI Background Check
  • 90 clock hours of training
  • For each assistant
    1. NAFCC Health Assessment Form
    2. NAFCC TB Screening Form
    3. Current First Aid
    4. Current Pediatric CPR
