First Aid and Pediatric CPR

Providers and assistants must submit a current first aid and pediatric CPR certification.

Please submit copies of the front, back and information we may need to determine that the training meets the eligibility requirements.

For example, many cards have circles that are filled in (or empty) to indicate which modules were completed. We need to be able to determine if the proper training was completed.
